Okay, I think there are two important themes here and the first is prison, or the feeling of being trapped. We start off with Matt reflecting that he feels he's in a "prison he couldn't bring himself to escape from" and this theme of being in a self induced state of imprisonment is something that runs throughout the entire fic. Matt can leave the relationship with Jessica, Danny and Luke but he doesn't want to, just as he can just quit being a superhero but he can't bring himself to. The imagery of Matt being fenced in, as if in a prison, opens the fic:
He sat on the edge of the bed, Danny and Jessica and Luke curled up together on the bed behind him.
Here we see the bed and his lovers acting as cell imagery, with Matt perched on the edge of the bed as if looking through the bars of his relationship's cell, able to see (FIGURATIVELY I HAVEN'T FORGOTTEN HE'S BLIND AGAIN) freedom but not able to chase it.
Which brings us to the other theme of the fic, exhaustion and weariness. First we see that Matt is physically tired - he can't sleep, the fic tells us - but we go on to see that he is also emotionally tired, exhausted by the prison he's created for himself. The choice whether to sleep or to prolong his exhaustion for the sake of others is presented, but Matt never considers which answer is the right one, and the very fact that this is never brought up highlights two things: Matt is too tired to even think about moral dilemmas like that, but more importantly for him it's obvious which answer is right. As the problem is never truly discussed, the audience is no chance to make their own decision, but are instead pulled along by Matt's own mindset on the matter, effectively also momentarily imprisoned as Matt is by Matt's own point of view.
